Job Role Summary

The TeleHealth Registered Nurse upholds the essential function(s) of directing and maintaining the efficient workflow of all Connections’ triage nurse operations. These functions include, but are not limited to, assessing needs and clinical options of patients, facilitating referrals to various primary care providers, specialists, healthcare facilities, or community resources, and performing a variety of operational, clerical, and patient focused service duties. This position independently functions as a practitioner while providing clinical assessments. This positon demonstrates problem solving, decision-making, and critical thinking skills. This position is patient and employee facing within the Eskenazi Health system and the role requires that the TeleHealth Registered Nurse always maintain positive customer and employee interactions. This position reports to the Connections’ Integrated Care Supervisor, RN 

Essential Functions and Responsibilities

•    Proactively contributes to Eskenazi Health’s mission: Advocate, Care, Teach and Serve with special emphasis on the vulnerable population of Marion County. Models Eskenazi Health values.
•    Triages and documents incoming calls from patients.
•    Assesses needs and clinical options, and facilitates referrals to various primary care providers, specialists, healthcare facilities and community resources.
•    Independently functions as a practitioner while providing total assessments. Demonstrates problem solving, decision making and critical thinking skills. Initiates actions to ensure continuity and effectiveness of patient care.
•    Performs the role of Charge Nurse, and Supervisory Nurse
•    Establishes and reviews priority order for his/her own activities and others under his/her supervision, in addition to all call center activities.
•    Recognizes and intervenes appropriately in situations that place callers/patients/families at risk; provides rationale/explanation for care advice given.
•    Ensures telephone triage nursing practice is regulatory compliant.
•    Demonstrates understanding of pharmacology.
•    Advocates and maintains patient/individual rights.
•    Alters activities/behavior to ensure adequate age-appropriate care to patients served (i.e. adolescent, geriatric, pediatric, neonatal, and adult).
•    Recommends the suggested medical follow-up and provides health information to educate the patient/caller in his/her healthcare.
•    Schedules patient appointments in EPIC system.
•    Utilizes interpersonal communication strategies/skills to achieve desired outcome with patients/families and others.
•    Responds to others in a positive, constructive, timely and courteous manner.
•    Functions effectively as a team member and responds to change in a constructive manner.
•    Responsible for self-learning and supports learning experience for others.
•    Assists in answering incoming calls.

Job Requirements

•    Minimum 4 years clinical nursing experience.
•    Must be proficient in computer use.
•    Active Indiana Registered Nurse license

Must live within an hour of Eskenazi Health downtown location

Knowledge, Skills & Abilities

•    Effective communication skills, knowledge of disease processes, and normal growth and development for all age groups in order to ascertain an accurate understanding of the client’s symptoms.
•    Must demonstrate strong listening skills to identify the level of comprehension and the verbal clues that patient is giving regarding pain, anxiety and fear.
•    Critical thinking skills plus the ability to handle stressful situations, and the capacity to function independently.
•    Must have varied clinical experience.
•    Ability to document meticulously; detail-oriented.
•    Must be able to determine the severity of the caller’s complaint using a series of algorithms developed though a coordinated effort of physicians and nurses. Direct the caller to the appropriate emergency services as necessary, recommends the suggested medical follow-up based on their assessments and established triage protocols, and provides health information.
•    May be required to complete follow-up calls to high-risk patients to assess changes of status or to ensure that the patient sought the appropriate treatment.
•    Must pass annual competency exams as required
•    Must be able to prioritize job functions so that the most critical issues are handled first. 
•    Must be able to multitask
•    Must have good communication skills when dealing with superiors and physicians.
•    Must have good technical and typing skills
