Job Role Summary
The Senior Case Management Technician is responsible for receiving and investigating communicable disease reports (CDRs) from the Indiana State Department of Health and providers throughout Indiana STD Reporting District 5. Case Management Technicians investigate CDRs without documented treatment by searching electronic medical records and/or following up with the ordering physician. If treatment cannot be verified, it is the responsibility of this position to assign the case to Disease Intervention Specialists (DIS) for follow-up. In addition to the normal case management tasks, the Senior Case Management Technician will serve as the point of contact for all Gonorrhea (GC) and Chlamydia (CT) inquiries from other Indiana DIS districts. This position will also include helping assist the other Case Management Technicians with difficult cases, reporting/surveillance issues, and training.
For those reports that come directly to the Marion County Public Health Department, the person(s) in this position are responsible for entering positive lab tests for Gonorrhea, Chlamydia, and Syphilis from Bell Flower Clinic and all area providers.
Essential Duties
- Checks NBS (NEDSS based Reporting System), electronic reports in Insight, LIMS and paper reports for cases of reported disease to see if GC and CT have been treated. NBS reports are in the Open Investigations Queue or in the Documents requiring Review Queue.
- Serve as the point of contact for other DIS districts in regards to GC/CT questions
- Investigate case report by searching available databases and electronic medical records including Care Web, Epic, and Insight or by calling providers. For each patient found, search for treatment.
- If a case has been treated, and treatment is documented, create morbidity and close the case.
- If treatment is not in NBS and is found by the procedures in “3. then treatment is added to NBS, and morbidity is created and the case closed.
- If treatment is not documented within 7 days from the date of the positive lab test, the case will be assigned to a Disease Intervention Specialist for follow-up.
- Generate morbidity for all GC’s and CT’s.
- Keep tic sheet of case assignment to DIS.
- Participates in public health emergency preparedness exercises and in the response to public health emergencies, as directed
- Participates in a minimum of two H.O.P.E. events totaling a minimum of eight (8) hours annually
- Assists other Case Management Technician with issues in surveillance, provider reporting, and new/ongiong training.
- Other duties as assigned.
Qualifications
- High school diploma or equivalent required, an advanced degree is highly desired;
- 1-2 years previous office coordination experience required; and/or training; or equivalent combination of education and experience.
Knowledge, Skills & Abilities
Language Skills:
- Must be able to effectively communicate verbally and in writing with a wide variety of staff at various levels of knowledge and communication, as well as members of outside organizations/agencies, and the public; ability to display patience, understanding, initiative, tact and maturity and the ability to communicate concepts and instructions clearly.
Mathematical Skills:
- Ability to add and subtract two digit numbers and to multiply and divide with 10’sand 100’s.
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
- Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning Ability:
- Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
- Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form
Working Enviornment
- Standard medical office environment
- Exposure to communicable disease, illness and combative patients
- Some physical effort associated with carrying of supplies for health fairs and handling clinic supplies
