Job Role Summary

The RN Specialist - Renal Care maintains and applies specialized evidence based clinical knowledge and expertise to ensure current practice standards and patient care processes are in place; establishes and facilitates patient care goals with patient, family, and health care team to achieve optimal outcomes, by providing education, coordinating care, and monitoring progress. This position serves as clinical consultant, educator, practitioner, resource and care coordinator to facilitate quality patient care and achieve optimal care outcomes for outpatient renal services.

Essential Functions and Responsibilities

  • Functions as a clinical nurse providing focused professional renal nursing care in the outpatient setting
  • Serves as clinical consultant, educator, practitioner, resource and care coordinator to facilitate quality patient care and achieve optimal care outcomes for outpatient renal services
  • Maintains and applies specialized evidence based clinical knowledge and expertise to ensure current practice standards and patient care processes are in place; establishes and facilitates patient care goals with patient, family, and health care team to achieve optimal outcomes, by providing education, coordinating care, and monitoring progress
  • Collaborates with renal team to achieve organizational objectives/goals for outpatient clinical program
  • Provides direct care/interventions to identified clients and families which may include but are not limited to: in-depth patient and family education, patient and family counseling, in-depth patient assessment, and documents in the medical record
  • Consults and/or intervenes in actual/potential situations that place patients at risk to minimize/correct/prevent risk within focused specialty; works directly with patient and family in coping with actual and/or potential risk
  • Adapts care and interventions to meet patient care needs in relation to age, size, language, and culture; modifies own and program interventions as needed for staff to meet these assessed needs as well
  • Assists in development and management of outpatient care programs
  • Independently performs patient education both during clinic and at nursing visits
  • Coordinates with vascular clinic to enhance access formation
  • Develops access surveillance plan with service line director and clinic faculty
  • Assists with nutrition consults and ongoing education
  • Assists Social Worker and Physicians in outpatient dialysis initiation for stable patients
  • Assists with admissions from clinic to hospital
  • Assists clinic staff during Renal Clinic to promote optimal patient experience and throughput
  • Assesses and coordinates care for patient to optimize thru-put, collaborates with healthcare team, medical staff and ancillary staff to effectively and efficiently achieve optimal care goals
  • Reviews, develops and evaluates policies/protocols procedures/practices and patient care programs incorporating evidence based practice concepts related to outpatient renal services
  • Formulates, evaluates and provides as needed, education/discharge planning related to potential and/or identify problems for patients, implements teaching programs for patients and families

Job Requirements

  • BSN from a professional school of Nursing
  • Minimum of three years of clinical nursing experience
  • Current Indiana license as a Registered Nurse

Knowledge, Skills & Abilities

  • Demonstrates clinical expertise in nursing techniques/methods/theory, competence and accountability in relation to care of renal patients
  • Ability to integrate evidence based practice into own practice and any policy, procedure, standards and/or protocol review or development
  • Knowledge and skills necessary to provide population specific care. Must integrate knowledge of the growth development principles over the life span and possess the ability to assess data/interpret appropriate information needed to identify each patient's requirements relative to age-specific needs. Ability to use age-appropriate communication and cognitive skills detailed in the department/area/unit job specific/age specific competencies
  • Excellent verbal communication skills required
  • Ability to:
    • Organize, manage and evaluate workload of self and other unit/health care personnel, prioritize duties and patient needs and assists others in doing so
    • Initiate, teach, evaluate planning and the provision of total patient care and effectively respond to stress
    • Function independently, utilizing clinical expertise to evaluate patient care
    • Critically think/problem solve care issues at the system level
