Job Role Summary
This position is responsible for providing specialized professional healthcare services which include confidential medical treatment and primary care for patients, with particular focus on sexually transmitted infections (STIs,) to patients under the supervision of and in collaboration with designated physicians and nurses. It ensures the continuity of patient care through health maintenance activities including the evaluation/early detection/management of health problems and appropriate referrals.
Associated Job Duties
- Assesses the physical/psychosocial status of patients, by obtaining medical history, performing physicalexam, and completing Risk Assessment which includes sexual history, substance use, domestic violence,sexual abuse history, housing, and history of incarceration.
- Records physical examinations, reviews charts and test results for abnormalities; makes judgementsregarding problem disposition, further evaluation, and referrals.
- Obtains, as needed, blood samples by phlebotomy, and/or specimens for STD testing from genital, oral orrectal sites.
- Establishes a partnership with the patient that facilitates decision making and selfcare consistent with thepatient’s health belief system while providing counseling and education that matches the individual’sunique needs, lifestyle, and personal readiness to change.
- Determines which patients: can be served by staff personnel; require physician attention or consultation;may refer to other agencies/resources for services.
- Initiates and evaluates diagnostic studies and plans therapy.
- Develops treatment plan with patient; determines types of services, including need of sex partnertreatment and/or notification, with goals and objectives of these services.
- Administers medications in accordance with physician prescribed orders and protocols.
- Performs referral, follow-up procedures to ensure continuity, compliance with healthcare/prescribed
- therapeutic regimens, medications, etc.
- Executes regimens as prescribed by a physician with unlimited license to practice medicine.

- Maintains appropriate records, and uses established medical record, forms/EMR, databases and documentation practices of organization.
- Reviews/monitors/guides health care activities of appropriate personnel.
- Works as a member of a team of health professionals within specific guidelines to identify, clinically manage, and evaluate patient medical problems.
- Assumes responsibility for professional self-growth and continued development in nursing by pursuing education, participating in professional work groups and committees.
- Ensures that all personnel actions are in accordance with Health and Hospital Corporation policies and procedures.
- Participates in public health emergency exercises and in the response to public health emergencies, as directed.
- Participates in a minimum of two HOPE events totaling a minimum of 8 hours annually.
- Performs other duties as assigned.
- Must be able to lift up to 25 pounds.
- Must be able to perform job duties in environment with moderate noise level (examples: business office with computers and printers, light traffic). Has exposure to communicable diseases; combative patients in stress situations.
- Must be able to stand, walk, use hands to handle or feel, reach with hands and arms, and talk or hear over __ of the on-the-job time.

Qualifications
- Fifth year of college or university program certificate; or two to four years related experience and/or training; or equivalent combination of education and experience.
Licenses/Certifications Required
- Current Indiana license as Registered Nurse.
- Completion of a MSN degree from an NLN/NCC accredited program that includes nurse practitioner training in one of the following: OB/GYN, Pediatrics/Family Practice, or successful post graduate completion of a formal nurse practitioner educational program in OB/GYN, Pediatrics, or Family Practice in a university setting.
Knowledge, Skills & Abilities
- Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulation. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
- Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Ability to use Microsoft Office Products or the equivalent. Ability to navigate Windows Explorer and e-mail.
- Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions.

- Some physical effort associated with carrying of supplies for health fairs and handling clinic supplies.
