Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 333-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus including at a network of Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary

The Lifestyle Wellness Coach provides patient education, peer support, and advocacy to promote optimal wellness in all stages of life. As part of the lifestyle health team, the Lifestyle Wellness Coach supports patients in setting and achieving their wellness goals via one on one health coaching, support groups, lifestyle medicine group programs, and resource facilitation.  Proactively contributes to Eskenazi Health's mission: Advocate, Care, Teach and Serve with special emphasis on the vulnerable population of Marion County while modeling Eskenazi Health PRIDE values.

Essential Functions and Responsibilities

•    Provides effective, safe and appropriate lifestyle health, wellness, nutrition, fitness, and health education based on current evidence based practice models.  
•    Utilizes motivational interviewing and behavior change skills to assess the overall wellness of patients and assists them in developing tailored wellness lifestyle goals. 
•    Creates goals collaboratively with patients, giving consideration and resources relative to social determinants of health.  
•    Establishes and maintains rapport and regular (minimum bimonthly) contact with patients
•    Supports, recruits, leads, and establishes Lifestyle Medicine programming as part of the multidisciplinary team, including serving as site lead for CDC Diabetes Prevention Program activities. 
•    Accurately uses electronic medical record to document patient contact. Maintains accuracy in documentation, along with scheduling and referral management, while meeting established productivity metrics and assigned deadlines.
•    Establishes effective working relationships with providers and open communication with multidisciplinary teams to assure continuity of care for patients. 
•    Appropriately promote lifestyle programs to providers and patients and external contacts in the surrounding community monthly through building relationships, word-of-mouth, and written material
•    Develops and implements a process improvement project or program review annually.  
•    Provides education to staff on hot topics and assist with onboarding of new staff.  
•    Organize and lead a minimum of one clinical, community, or professional activity annually 
 

Job Requirements

•    Bachelor’s Degree in Health Promotion, Exercise Science, or other health related field; Master’s Degree preferred.  In lieu of Bachelor’s degree high school diploma/GED will be acceptable with proven 10 years of similar work experience.
•    CDC Lifestyle Coaching Certification and/or other fitness/coaching certifications required 
•    Bilingual (English/Spanish) highly preferred 
 

Knowledge, Skills & Abilities

•    General knowledge of word processing, spreadsheet software and other computer programs as required for patient documentation, patient health information, and departmental related reports and projects
•    Excellent written and verbal communication skills in dealing with diverse patient populations and health care team members
