Eskenazi Health is seeking a compassionate and skilled Licensed Clinician to join our Primary Care team. In this role, you will provide integrated behavioral health services, ensuring patients receive comprehensive, person-centered care that promotes mental and physical well-being.

Essential Functions and Responsibilities

What You’ll Do:

  • Complete biopsychosocial evaluations (BPSE) and develop integrated diagnostic summaries.
  • Prepare and update clinical treatment plans in collaboration with clients, families, and the treatment team.
  • Manage a caseload, identifying therapeutic, rehabilitative, and case management needs.
  • Evaluates and updates the treatment plan at intervals determined by guidelines, consulting with client family and team in this process. 
  • Deliver individual, family, and group therapeutic interventions using best practices for diverse populations.
  • Advocate for clients by connecting them to community resources and services.
  • Monitor client progress and maintain accurate, timely documentation in compliance with agency standards and reporting to treating psychiatrist.
  • Identify high-risk symptoms or circumstances and consult with senior clinical staff as needed.
  • Serve as a clinical resource for less experienced staff and actively participate in supervision for professional growth.
  • Facilitate community groups, develop resources, and provide education to external agencies.
  • Ensure billing accuracy and compliance with service guidelines.

Qualifications:

  • Master’s degree in Social Work, Counseling, or related field.
  • Current licensure as LCSW, LMHC, or LMFT in the state of Indiana.
  • Valid Indiana Driver's license that meets Eskenazi Health's driving policy requirements.
  • All staff working with clients from Department of Child Services will be required to be fingerprinted and have a criminal background check performed upon hire and periodically thereafter.
  • All individuals working for the Opioid Treatment Program must have one of the following FSSA/DMHA-approved credentials for Clinicians providing counseling with an OTP:
    • Licensed Clinical Social Worker (LCSW)
    • Licensed Marriage & Family Therapist (LMFT)
    • Licensed Mental Health Counselor (LMHC)
    • Certified Alcoholism and Drug Abuse Counselor (CADAC) II
    • Certified Alcoholism and Drug Abuse Counselor (CADAC) IV
    • Indiana Certified Addiction Counselor (ICAC) I
    • Indiana Certified Addiction Counselor (ICAC) II
    • Licensed Social Worker (LSW) with Medication Assisted Treatment Specialist (MATS)
    • Licensed Clinical Addiction Counselor (LCAC)
    • Licensed Addiction Counselor (LAC)
    • Master Addiction Counselor (MAC)
    • National Certified Addiction Counselor (NCAC) I
