Job Role Summary

The Lead Poisoning Case Manager is responsible for providing comprehensive, state-mandated case management services for children identified with lead exposure. The Case Manager will adhere to CDC guidelines, maintain current knowledge of lead poisoning interventions, and collaborate with program partners to enhance blood lead testing and link affected families to essential services. The role involves developing public awareness strategies and participating in community initiatives focused on lead poisoning prevention. The Case Manager will work closely with the Lead Poisoning Risk Assessor.

Essential Duties

Home Visits and Education 40%

  • Conduct home visits to families where a lead-poisoned child resides, providing education on the impact of lead on health, routes of exposure, and potential sources of lead in the home.
  • Ensure all siblings in the home under the age of 7 are tested for lead exposure.
  • Conduct developmental, nutritional assessments on lead poisoned children.
  • Complete appropriate referrals to Marion County Public Health Department and/or outside agencies if necessary. 

Documentation and Compliance 10%

  • Ensure compliance with retesting guidelines and report blood lead test results to primary care physicians.
  • Explain and obtain signed copies of releases of information, notices of privacy practices, and any other required paperwork.
  • Maintain proper documentation in appropriate databases. 

Community Engagement and Education 20%

  • Attend and actively participate in neighborhood meetings and health fairs, preparing necessary reports for these gatherings.
  • Provide educational services to community members regarding lead poisoning prevention and awareness.
  • Conduct or participate in periodic surveys and door-to-door evaluations to assess the environmental health status of neighborhoods and communicate the results to the community served
  • Maintain current knowledge on lead poisoning issues and interventions.

Associated Job Duties

Collaboration and Coordination 5%

  • Coordinate and collaborate with other public health departments and governmental agencies to enhance service delivery and community health outcomes.
  • Participate in public health emergency preparedness exercises and respond to public health emergencies as directed.
  • Perform other duties as assigned.

Qualifications

Bachelor’s degree required in public health, social work, sociology or related field.

Licenses/Certifications Required

  • Valid and current driver’s license, good driving record, current automobile insurance, and access to reliable personal transportation for work purposes.

Knowledge, Skills & Abilities

  • Strong verbal and written communication skills.
  • Computer proficiency in Microsoft Word, Excel, Power Point, and web based.
  • Self-starter capable of working independently.
  • Motivated and able to shift daily work plan as priorities change.
  • Effective coordinator with internal staff and key outside constituents.
  • Ability to organize and prioritize work and meet strict deadlines.

Working Environment

  • This position requires significant travel and work outside the base office and inside client homes.
  • Professional settings may include healthcare facilities throughout the county.
  • Office time includes prolonged sitting, computer screen use, and steady use of hands and fingers.
