Job Role Summary
The Hepatitis C Virus (HCV) Care Coordinator will support people living with HCV by providing them with care coordination and linkage to medical treatment. This position will assess eligible patients for needs, identify barriers to hepatitis treatment, and then connect patients with resources to reduce or eliminate those barriers. The HCV Care Coordinator will work closely with other departments, community resources, and clinics to help their patients reduce barriers and receive curative treatment.
Essential Functions and Responsibilities
- Initiates contact with patients diagnosed with HCV to facilitate linkage to care and treatment.
- Provides case management services with the goal of curing HCV with a Sustained Virological Response (SVR).
- Assists patients with applying for medical insurance, patient assistance or other programs that support HCV treatment and cure.
- Provides patient education about Hepatitis B virus (HBV), HCV, Human Immunodeficiency virus (HIV), and/or Harm Reduction based on patient need.
- Tests or facilitates laboratory testing for HBV, HCV, HIV, and/or related testing.
- Refers for HBV vaccination, other medical and/or social services as appropriate.
- Develops and maintains a strong working relationship with the emergency department (ED), primary care and specialty care clinics for referral and collaboration.
- Builds and maintains working relationships with local providers and other community partners who work with the focus population.
- Identifies and develops innovative strategies and community partnerships to address linkage to care barriers.
- Monitors and ensures patient engagement in care from testing to cure by sending appointment reminders, following up with those who miss appointments, and frequently checking in with patients about their needs.
- Refers clients to additional services such as Sexually Transmitted Infection (STI) treatment, substance use treatment, and supportive social services as needed.
- Assists patients with sustainability planning and extended care plan for ongoing liver monitoring, upon achievement of sustained virologic suppression.
- Travel to, from, and between multiple Eskenazi Health locations and workspaces.
- Documents all patient encounters in Epic and other reporting systems as dictated by grant funding.
- Develops and maintains reports within Epic, Excel, and other applicable databases.
- Develops and implements evaluation tools and activities for quality improvement of program initiatives.
- Participates in testing and outreach events as needed.
- Attends regularly scheduled calls and training activities hosted by the Damien Center, The Health Foundation of Greater Indianapolis, and/or the Indiana Department of Health.
- Provides accurate data for quantitative and qualitative reports for grant as requested.
- Performs special assignments and other duties, on an as-needed basis
Job Requirements
- Bachelor's degree in social work, Public Health, or related field.
- Persons with lived experience with HCV are encouraged to apply.
- Experience working with diverse populations preferred.
- Valid Indiana Driver’s License
Knowledge, Skills & Abilities
- Basic knowledge of Hepatitis B, Hepatitis C, and harm reduction strategies and passion to expand knowledge in these areas.
- Aptitude for working with vulnerable populations, including staff, patients, and support systems, regardless of age, race, gender, sexual orientation, and substance use history.
- Ability to work independently while achieving exacting standards of productivity, meeting deadlines, and producing results.
- Capable of operating a personal computer with knowledge of Microsoft Office, especially Excel, Word, and Outlook.
- Strong organizational, time management and document management skills.
- Skilled in critical thinking, motivational interviewing, and other applicable theories.
- Excellent verbal, written, and interpersonal skills.
- Ability to supervise workloads independently.
