Job Role Summary
Responsible for providing a continuum of social work services in an assigned work setting which will assist patients/clients and their families to find practical ways of resolving social, emotional and environmental problems that interfere with optimum functioning and attainment of total health. Works closely with the Bureau of Environmental Health/Housing & Neighborhood Health Department and other community service providers to maximize health outcomes for individuals, families and communities. The majority of services are home-based.
Associated Job Duties
- Serve on appropriate committees and task forces as assigned.
- Trained at the appropriate National Incident Management System (NIMS) and Incident Command System.
- Training on Psychological First Aid (PSA) in preparation to participate in the response to public health disasters/emergencies.
- Participate in Health Fairs.
Qualifications
Education/Licensure/Certifications/Experience:
- Must be a Licensed Social Worker (LSW).
- Obtain LCSW license when eligible.
- Master Degree in Social Work.
- A valid Indiana Driver's License and access to a vehicle is required.

Knowledge, Skills & Abilities
Analytical, psychosocial assessment and counseling skills acquired through:
- Graduation with Masters in Social Work Degree from an accredited School of Social Work.
- 1-3 years of social work/counseling experience in a healthcare setting is preferred.
- Knowledge of pertinent State/Federal laws and regulations, community systems and resources.
- Ability to work cooperatively with multi-disciplinary teams in situations requiring a high degree of flexibility, creativity, initiative.
- Apply social work/counseling techniques/principles including evaluative, intervention, counseling methods; toward productive problem solving in the client/worker relationship.
- Communicate effectively with a wide variety of people and perform duties independently in a wide variety of settings.
- Knowledge and use of computer and skills for writing/documenting case notes, reports, e-mails, etc.
