Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 333-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus including at a network of Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary
Essential Functions and Responsibilities
- Provides services to persons diagnosed with HIV/AIDS and their significant others
- Assists persons infected/affected with HIV with gaining access to medical, psychosocial, social, financial and other related services as an ongoing process
- Performs care coordination services that include: comprehensive assessment, individualized care plans, providing linkage to appropriate community resources, monitoring services received, following individual progress and adapting care plans.
- Works in collaboration with other service/agency providers for persons infected with HIV/AIDS; provides service for individuals regardless of age, race, religion, gender, beliefs, culture, mental and physical ability and sexual orientation
- Completes comprehensive psychosocial screening and medical history on all new HIV patients being followed by Eskenazi Health Infectious Disease
- Facilitates the completion of the care coordination agreement and release of information agreements
- Participates in multidisciplinary staffing on clinic days
- Meets with patients in clinic and prepares timely documentation for discussion and referrals on standardized clinic encounter forms
- Refers clients to community resources for financial assistance, assists clients in filling out applications for emergency financial assistance: DEFA (Direct Emergency Financial Assistance)
- Links clients to community resources for basic needs ie., food, clothing and shelter
- Collaborates with agencies in the community who also serve HIV clients
- Counsels clients on end of life issues
- Attends educational trainings as required by Grantees (federal, state and county)
- Performs grant writing for care coordination position
- Consults with HIV patients admitted to the hospital and provides information and resources pertaining to HIV/AIDS with complete assessment documented in the history and physical.
- Refers to inpatient social worker for discharge planning
- Alters activities/behaviors to reflect/ensure adequate care appropriate of age of patients served (i.e. adolescent, geriatric, pediatric, neonatal, adult) as described in the area/unit age-specific and/or job specific competencies
Job Requirements
- Master's degree in social work (may be obtained after hire if already enrolled per the department's discretion)
- Current LSW or LCSW
- Social work experience in a health care setting is preferred
Knowledge, Skills & Abilities
- Knowledge of the HIV disease process
- Maintains ability to work independently and on an interdisciplinary team
- Ability to:
- Bill Medicaid for Care Coordination services provided to clients who have Medicaid insurance
- Assess, identify and utilize community resources to meet individual needs
- Assist in filling out applications for insurance programs such as: Medicaid, ADAP, ICHIA, HIAP, Ryan White Service Program, referrals to financial counselor for Eskenazi Health Advantage program, and provides information about insurance programs
- Provide direct and indirect advocacy for HIV patients
- Provide educational initiative aimed at patients, significant others, hospital staff and the community focused on HIV
- Assist clients on completing legal forms for Power of Attorney, Living Wills and Health Care Representative
- Write statements and to gather information on HIV/AIDS statistics, HIV/AIDS populations and services provided for clients
- Communicate with patients, family and staff with sensitivity for this vulnerable population
