Job Role Summary

This position is responsible for contacting and meeting with all prospective clients to determine eligibility for intensive home visitation services. Contact all at risk families referred to the Healthy Families program and utilize the identified assessment tool, the Kemp Family Stress Checklist to determine and assess level of risk. Document all interactions with clients on a centralized database. Inform clients about various resources in the community with specific emphasis on Healthy Families Program.  Refer families to the appropriate resources specific to their needs.  Encourage and support at risk families to build positive parenting skills. Market Healthy Families program in the community.


  • Bachelor's degree in social work, psychology, sociology, child development, family studies or related field required.
  • 1 to 3 years previous work with families and children (0- 3 years of age) preferred.

Licenses/Certifications Required

  • Valid driver's license
  • Valid automobile insurance

Knowledge, Skills & Abilities

  • Must have experience with and/or willingness to work with the culturally diverse populations present among the program's target population.
  • Must have knowledge of infant and child development.

Working Enviornment

  • Unconfined sitting, working at computer, attending meetings, etc.; standing and walking; steady use of hands and fingers – data entry; limited lifting and carrying.
  • May be exposed to substandard and unsanitary living conditions. 
  • May be exposed to allergens of all types (i.e. pets, dust, smoke, etc.)
  • Must have valid Indiana Driver’s License, access to own working vehicle, current automobile insurance
