Job Summary

Transform Healthcare. Improve Outcomes. Drive Meaningful Change.

Eskenazi Health is seeking an innovative and strategic Director of System Value-Based Care to lead the growth and performance of our value-based care (VBC) program. This leadership role will unite population health, clinical operations, quality, pharmacy, analytics, payer strategy, and community health efforts into a coordinated approach that improves outcomes, enhances patient experience, and lowers the total cost of care.

This leader will help accelerate Eskenazi Health's transition from encounter-based care to a proactive, data-driven model focused on prevention, chronic disease management, and whole-person health. With direct accountability for value-based performance, the Director will build the infrastructure, partnerships, and operational capabilities necessary for success in risk-based arrangements while evaluating emerging technologies, including AI-enabled solutions, that support better outcomes and greater efficiency.

Key Responsibilities

  • Lead the development and execution of Eskenazi Health's value-based care strategy.
  • Align clinical, operational, financial, and contracting teams around shared goals and measurable outcomes.
  • Establish governance and accountability structures that drive value-based performance.
  • Strengthen care models focused on prevention, care gap closure, access, continuity, and chronic disease management.
  • Partner with primary care, care coordination, behavioral health, pharmacy, and community health teams to improve outcomes for risk-stratified populations.
  • Advance care transition and medication management processes that reduce avoidable utilization and improve coordination.
  • Collaborate with executive leaders on payer strategy, value-based contracting, and performance evaluation.
  • Guide organizational readiness for increasing levels of financial risk.
  • Oversee financial performance across value-based contracts, including total cost of care and shared savings opportunities.
  • Leverage data, analytics, and technology to support performance improvement and decision-making.

Qualifications

The ideal candidate will possess an advanced degree in healthcare administration, public health, business, medicine, nursing, or a related field (MBA, MHA, MPH, MD, DO, or equivalent) and at least 10 years of progressive leadership experience in value-based care, population health, managed care, accountable care, or payer strategy. Candidates should have expertise in value-based reimbursement models, including shared savings, capitation, risk adjustment, HCC coding, quality measurement, and total cost of care analysis, and a demonstrated ability to influence stakeholders across complex organizations.

Experience serving Medicaid and underserved populations is preferred, along with familiarity with FQHCs, safety-net health systems, and community-based care models. The successful candidate will have a proven track record of building or advancing value-based care programs, including responsibility for payer strategy, contract performance, operational execution, and financial outcomes.
