The Sandra Eskenazi Mental Health Center offers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

The Focus of the Assertive Community Treatment Team
The Assertive Community Treatment (ACT) Team provides intensive, community-based wraparound services to adults living with serious mental illness. Individuals served by ACT often have histories of psychiatric hospitalization, incarceration, housing instability, crisis service utilization, or other complex challenges that require a high level of coordinated support.
ACT is a recovery-oriented, multidisciplinary team dedicated to helping some of Marion County's most vulnerable individuals achieve stability, independence, and personal recovery goals. Team members work collaboratively to provide individualized care and respond to clients' evolving needs, while fostering long-term growth and community integration.
This role is primarily community-based and involves meeting clients where they are, whether in their homes or other community settings, while coordinating care with a highly integrated treatment team. The ACT team includes psychotherapists, psychologists, psychiatric nurses, care coordinators, peer recovery specialists, employment specialists, access coordinators, clinical leadership, and other behavioral health professionals. With shared caseloads and a team-based approach, ACT offers a highly supportive and well-resourced environment focused on delivering meaningful outcomes for both clients and staff.

As a care coordinator, you can expect to complete case manager functions like conducting assessments, teaching life skills, documenting client progress, and locating resources to remove any barriers clients might be facing.
What To Expect
As a care coordinator on our ACT team, you’ll spend most of your time in the community meeting clients where they are, including in their homes, residential facilities, boarding homes, and other community locations. You will support adults living with severe mental illness, with a focus on individuals who have recently been hospitalized or have complex needs that are best addressed through intensive community-based care.
Am I Qualified?
- Bachelor’s degree required
- If incumbent holds a bachelor’s of social work degree, incumbent must also possess a current, unencumbered Licensed Bachelor’s Social Worker license in the State of Indiana
- Valid Indiana driver’s license and driving record that meets Eskenazi Health’s driving policy requirements

Essential Functions and Responsibilities
- Embraces, understands, and operates under the Recovery Model, including AIDET (Acknowledge, Introduce, Duration, Explanation, Thank You) and the spirit of motivational interviewing
- Provides skills training and development to clients in group and individual settings as guided by medical necessity outlined in the person-centered treatment plan
- In collaboration with the treatment team, the Care Coordinator conducts necessary client assessments as well as revises treatment plans
- Provides crisis management and emergency interventions including but not limited to cardiopulmonary resuscitation and approved area-specific crisis intervention techniques
- Reports to treatment team in staffing including but not limited to successes and challenges, assessment of client progress, barriers to success, any level of care changes, etc.
- Meets the agency’s standard of providing the appropriate service at the right time, right place, and right frequency as determined by the client’s level of need
- Meets the agency’s standard of timely completion of all documentation including interpreting and applying service and billing procedures according to client needs as well as completing billing according to guidelines for timeliness and accuracy
- Establishes and maintains a safe environment at all times through on-going assessment of milieu/appointment environment and intervenes as indicated in person-centered treatment plan as well as by nursing or physician order
- Provides case management, including working with family, significant others and community supports to assist clients in meeting goals
- Meets agency requirements for continuing education
- May provide transportation in personal vehicle as directed by treatment team and guided by medical necessity; maintains a copy of Eskenazi Health’s vehicle-related accident reporting procedures in vehicle
