Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Center offers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

Job Role Summary

The Care Coordinator provides skills training and development to clients as required by medical necessity. In collaboration with the treatment team, the Care Coordinator conducts necessary client assessments and formulates, develops, and revises treatment plans. The Care Coordinator advocates for clients to ensure access to needed services, including developing community relationships and resources. In partnership with Wheeler Mission, the Care Coordinator provides onsite behavioral health support, care coordination, and linkage to services for individuals experiencing homelessness, helping to reduce barriers to treatment and promote engagement in care.

What Is Wheeler Mission?

Wheeler Mission is a community-based organization serving individuals experiencing homelessness through shelter, housing, recovery, and supportive services. This role partners closely with Wheeler Mission staff to provide onsite behavioral health assessment, care coordination, crisis support, and linkage to treatment for residents with complex behavioral health needs.

Essential Functions and Responsibilities

  • Provide onsite behavioral health outreach, assessment, engagement, and care coordination services to residents at Wheeler Mission, reducing barriers to treatment and access to care.
  • Meet with residents to assess behavioral health needs, explore treatment options, connect individuals with mental health services, and coordinate referrals to community resources.
  • Serve as a liaison and advocate between residents, Wheeler Mission staff, Sandra Eskenazi Mental Health Center providers, and community partners to ensure coordinated, wraparound care.
  • Conduct behavioral health assessments, participate in treatment planning, monitor client progress, and collaborate with the interdisciplinary treatment team to address barriers and support recovery goals.
  • Provide individual skills training, case management, crisis intervention, and emergency response services as clinically indicated and guided by medical necessity.
  • Collaborate with onsite medical providers and behavioral health staff to identify needs, develop support strategies, and ensure continuity of care across service providers.
  • Participate in Professionally Blended Street Outreach, Coordination of Care, and Complex Needs Workgroups to support individuals experiencing homelessness and complex behavioral health challenges.
  • Maintain timely and accurate documentation, billing, and reporting in accordance with agency standards, regulatory requirements, and person-centered treatment plans.
  • Promote recovery-oriented services by utilizing motivational interviewing, AIDET principles, and evidence-based practices to engage clients and support long-term stability.
  • Coordinate with families, support systems, and community resources to assist clients in achieving treatment goals and improving overall functioning.
  • Report client progress, challenges, treatment needs, and level-of-care changes to the treatment team and collaborate to ensure services are delivered at the appropriate time, place, and frequency based on client needs.
  • Maintain timely and accurate documentation, billing, and reporting in accordance with agency standards, regulatory requirements, and medical necessity guidelines.
  • Complete required continuing education and provide transportation assistance when clinically appropriate and directed by the treatment team.
  • Conduct behavioral health assessments, participate in interdisciplinary treatment planning, revise person-centered treatment plans as needed, and monitor client progress toward recovery goals.
  • Provide case management services, including coordination with family members, support systems, medical providers, and community resources to address barriers and promote successful outcomes.
  • Deliver crisis intervention, risk assessment, and emergency response services, including approved crisis intervention techniques, while maintaining a safe and therapeutic environment.

Job Requirements

  • Bachelor’s degree required
    • If incumbent holds a bachelor’s of social work degree, incumbent must also possess a current, unencumbered Licensed Bachelor’s Social Worker license in the State of Indiana
  • Valid Indiana driver’s license and driving record that meets Eskenazi Health’s driving policy requirements
