Job Role Summary

This is an internship for students working towards their bachelor’s in social work (BSW). Responsible for providing a continuum of social work services in an assigned work setting that will assist patients/clients and their families find practical ways of resolving social, emotional, and environmental problems that interfere with optimum functioning and attainment of total health. Works closely with other community agencies and service providers to maximize health outcomes for individuals, families, and communities. Services will be in the home, community, and office. 
This position is aligned with the NASW (National Association of Social Workers) Code of Conduct & Ethics for Professional Social Workers; the APHA (American Public Health Association) Social Work Section-Standards of Practice and Competency for Public Health Social Workers; and also, in adherence and provision per the 10 Essential Public Health Services Competency for a Medical Social Worker.

Essential Duties

Provides therapeutic social work intervention to diverse client system which may include but not limited to crisis intervention, community resource referrals, client advocacy, support, outreach, follow-up, education, and intra/inter agency consultation. 
Participates in a multidisciplinary team effort to coordinate/implement services that will ensure appropriate treatment plan continuity of care for all clients/patients served of care. 
Assumes advocacy/liaison/collaborative role for patients/clients via contacts with community agencies/personnel, hospital personnel, court disposition hearings, abuse cases, etc.; collaborates with inter/intra agency personnel involving referral activities. 
Maintain confidentiality of patient/client records and information in accordance with HIPAA guidelines.

Qualifications

Education/Licensure/Certifications/Experience:
Currently enrolled in a bachelor’s degree social work program from an accredited school.
A valid Indiana Driver’s License and access to a vehicle is required.
 

Knowledge, Skills & Abilities

1-3 years of social work experience in a healthcare setting is preferred.
Knowledge of pertinent State/Federal laws, regulations, community systems, and resources.
Apply social work techniques/principles including evaluation, and intervention; toward productive problem solving in the client /worker relationship. 

Working Environment

Possible exposure to communicable diseases, infections; and other hazards, involved in home office, clinic, and community site visitations.
Effective response to stress situations; and appropriate response in dealing with difficult patients/clients/families.
Occasionally required to testify in court cases (abuse/neglect, custody, environmental issues).
Patient/Client medical records cannot be transported home or to another work site without permission from the supervisor and in compliance with MCPHD HIPPAA guidelines and protocol.
