Job Role Summary

Monitors performance standards for the Indianapolis Healthy Start program core services (outreach, health education interconception care) to ensure quality services are provided by core service staff. Supervises the day-to-day activities of outreach, health education, and interconception care Community Health Worker staff. Assist with the following: consumer development and increasing consumer involvement in program activities; development of policies and procedures, and coordination of professional and consumer trainings. Provides leadership in resource development, communications, and identifies and engages various organizations and stakeholders in carrying out the mission of IHS. Mentors staff to endure that everyone is involved and passionate about the IHS program. Coordinates and participates in the selection of evidenced based health education materials. Coordinates project events, media campaigns, and preparation and publishing of the project newsletter.

Associated Job Duties

  • Maintains awareness of and engagement with other community organizations 
  • Monitor and evaluate the success of health education and outreach 
  • Develops key partnerships in IHS catchment area and surrounding area. 
  • Assists Project Manager  in researching and securing funds for community engagement 
  • Review and create health education, outreach and CHW  P&P to be uploaded into PolicyStat 
  • Responsible to assist with holding subcontractors accountable for SOW deliverables 
  • Provides training for core service staff as needed 
  • Assists in consumer development and increasing consumer involvement in program activities.
  • Develops performance improvement measures and procedures based on core service nee
  • A minimum of (2) Plan-Do-Study-Acts (PDSA)s per grant year
  • Monitors assigned staff’s performance on core service outcome measures. 
  • Assists community members in planning assist with carrying out the requirements of the grant 
  • Coordinates media campaigns and other program material revisions with PR staff  
  • Participates in the interview process for new core service staff 
  • Provides excellent and accurate communications with  participants and partners 
  • Works closely with Project Manager program objectives are being met 
  • Trained at the appropriate (NIMS) (CIS) level. 
  • Maintains confidentiality defined by HIPAA of 1996 
  • Participates in public health emergency preparedness 
  •  Participates in a minimum of two H.O.P.E. events  (8) hours annually 
  • Performs other duties as assigned as needed (8)

Qualifications

  • Bachelor’s degree in Public Health, Social Work, or Related Field; Master’s degree preferred.  Experience in supervision or related performance improvement experience and experience in providing community engagement, outreach and health education, and perinatal health services is required.

Working Environment

  • Must be able to lift up to 40 pounds.
  • Long periods of viewing the computer monitor and keyboarding.
  • Local travel to Community Health Centers, clinics and community agencies in the IHS Catchment area and Marion County. Must be available to work evening and weekend events as needed.
  • State travel to national conferences and conventions in Washington, D.C. and other hosting states.
