Job Role Summary
This position requires appropriate intervention and provision of services through client assessment, health education, coaching, referral monitoring, facilitation, and follow-up to pregnant women and their infants up to age 2. To interface with Indianapolis Healthy Start subcontractors to provide education training and technical assistance to community agencies, service providers, and the community at-large as it relates to infant mortality.
Associated Job Duties
- Provide comprehensive range of evaluation and educational procedures to pregnant women and their children in accordance with Healthy Start Scope of Work
- Process new enrollees into the program by conducting risk assessment for all clients for high-risk pregnancy with the high-risk assessment tool
- Conduct depression screenings for enrollees by utilizing the Edinburgh Depression Scale as an assessment tool
- Develop supportive relationships with participants
- Provide case management in a coordinated and culturally sensitive approach
- Inform clients of available resources to address medical needs, social needs, WIC, and provide care coordination services for her pregnancy. Also, customize a care plan for support services that may include education goals, etc.
- Supply clients with clinical and educational services in regard to Healthy Start objectives. Provide health education that includes the importance of prenatal care throughout a pregnancy, smoking cessation, drug and alcohol abuse, appropriate weight gain, SIDS, breast-feeding, folic acid/nutrition, immunizations, Early Periodic Screening Diagnosis Treatment (EPSDT), & the importance of healthy behaviors
- Attend various workshops and conferences to increase awareness and understanding of health-related issues and those related to infant mortality
- Accurately document all required information
- Attend Healthy Start meetings
- Maintains confidentiality of all client records and information as specified by the Health Insurance Portability and Accountability of Act of 1996
- Trained at the appropriate National Incident Management System (NIMS) and Incident Command System (ICS) level
- Participates in Public Health Emergency exercises and in the response to public health emergencies
- Participates in a minimum of two H.O.P.E. events totaling a minimum of eight (8) hours annually
- Perform other job-related duties as assigned.
Qualifications
Bachelor’s degree required, an advanced degree is highly desired; 3-5 years experience in case management, social services, or maternal and child health.
