Job Role Summary

The Data to Action Outreach Specialist is responsible for providing outreach and social services referrals for wrap around services (i.e., insurance, housing, employment, substance use disorder treatment) to individuals who have experienced a non-fatal overdose.

 

Associated Job Duties

Assessment

  • Conduct Naloxone trainings at MCPHD District Health offices.
  • Provide outreach and social services referrals for wrap around services to surviving families/children of those who suffered a fatal overdose
  • Conduct outreach to individuals who are prescribed Opioids and are at high risk of SUD.

Policy Development

  • Inform, educate, and empower people about substance of abuse issues. 
  • Mobilize middle and high school partnerships to identify and solve substance abuse problems.
  • Establish and maintain effective lines of communication, collaboration and liaison with the community, targeted populations, other program areas within the Marion County Public Health Department (MCPHD).

Assurance

  • Data collection completion and submission as required.


Evaluate

  • Plan, implement and evaluate intervention efforts.
  • Analyze data, program activities and outcomes.
  • Prepare monthly project quarterly reports as requested and required in an accurate and timely manner.

Qualifications

Skill and knowledge acquired through a Bachelor’s degree with two to five years’ experience in Criminal Justice or related social services field. Must have working knowledge of the criminal justice and social services systems.

Licenses/Certifications Required

Valid Indiana drivers’ license, access to transportation for in-county travel and ability to be classified as a “driver” per Health & Hospital Corporation guidelines.

Knowledge, Skills & Abilities

Language Skills

  • Effective interpersonal oral and written communication skills.

Mathematical Skills

  • Cognitive ability to add, subtract, multiply and divide in all units of measure.
  • Knowledge of spread sheet development and assessment.

Reasoning Ability

  • Ability to analyze data to identify problems and implement corrective actions. 
  • Work independently with minimal supervision.
